Thank you for taking the time to fill out this online form to better serve your child(ren) a safe, modified menu. This online form is for special diet requests due to food allergies, texture modified diets, and other food related disabilities. Please fill out the following text fields and upload the completed Special Diet Form. The Child Nutrition Department will do our best make accommodations and provide safe alternatives for your student.
